This website
If you reserve a place, ask a question, or book, we receive what you type into the form: usually your name, email address and phone number, and the membership you are interested in. We use it to reply to you and to take care of your request. We do not sell your information.
To understand which of our pages and campaigns brought you here, your browser keeps where you came from, such as the link or campaign you clicked and the first page you saw. It leaves your browser only if you choose to submit a form.
This site does not use advertising trackers. Any visitor analytics are switched on only after you agree to them.
Card payments are taken on our payment provider’s secure page, so your full card number does not pass through our website. Please do not put detailed medical information in a website form or in ordinary email; those are for getting in touch.
Your health information
When you become a patient or member, the health information we create about you is protected by HIPAA. Our Notice of Privacy Practices describes how it may be used and shared, and your rights, including to see and get a copy of your record, to ask for corrections, and to complain. The Paradigm Health app
Members use the Paradigm Health app to see their plan, results and messages. The app is part of the electronic medical record service provided by Paradigm Health Research & Education Enterprise, LLC (“PHREE”), which stores the health information in it on our behalf, as our business associate under HIPAA. What the app can use on your phone, how Health Connect data is handled, and how to delete your account are in the app privacy policy at phree.org/privacy. You can delete your account from the app, or by emailing drdavid@phree.org with the subject “Delete my account.” Your medical record is not deleted: Tennessee law requires a physician to keep it for at least 10 years after your last visit. For a child, it is kept until at least 1 year after they turn 18, or for 10 years, whichever is longer. Immunization records, and the records of a patient who cannot make their own decisions, are kept indefinitely, and nothing connected to a service that is in dispute is destroyed until the dispute is settled. Federal HIPAA law does not set its own period for medical records.
Children
This website is meant for adults, and we do not knowingly collect information from children through it.
